Delivering better services for people with long term conditions

This paper focuses on a co-ordinated service delivery model – the ‘house of care’ – that aims to deliver proactive, holistic and patient-centred care for people with long-term conditions.

The house of care model differs from others by encompassing all people with long-term conditions and assuming an active role for patients. This paper calls on the Department of Health and NHS England to collaborate on a coherent approach to care planning for people with long-term conditions.

Children and young people’s emotional health and wellbeing needs assessment

The factors that contribute to positive and negative emotional health amongst children and young people are explored under six priority area headings, developed by the Children’s Society. These include having ‘the conditions to learn and develop’ and ‘a safe and suitable home environment’. The needs assessment seeks to paint a picture of need across Merseyside.

Government published final report on Winterbourne View Hospital

The final report into the events at Winterbourne View Hospital states that staff routinely mistreated and abused patients, and management allowed a culture of abuse to flourish. The warning signs were not picked up, and concerns raised by a whistleblower went unheeded. The report also reveals weaknesses in the system’s ability to hold the leaders of care organisations to account. In addition, it finds that many people are in hospital who don’t need to be. People with learning disabilities or autism, who also have mental health conditions or challenging behaviour can be, and have a right to be, given the support and care they need in the community, near to family and friends.
