Transitions at the end of life for older adults – patient, carer and professional perspectives: a mixed-methods study – National Institute for Health Research (NIHR)
The aim of this study was to understand the experiences, influences and consequences of transitions between settings for older adults at the end of life. Three conditions, heart failure, lung cancer and stroke, were the focus of study, chosen to represent differing disease trajectories.

This prototype dashboard is designed to encourage CCGs and trusts to think more about how well their patients are supported to use medicine and less about focusing on cost and volume of drugs. It brings together data in one place and includes prescribing comparators, as well as looking at areas such as medication safety and utilisation of community pharmacy services.
