The Cardiorenal Syndrome : A Clinician’s Guide to Pathophysiology and Management

Cardiorenal syndrome – the presence of significant renal insufficiency and heart failure simultaneously – constitutes one of the most vexing and difficult challenges facing clinicians and researchers today. Given the fundamental role of the kidneys in maintaining fluid and electrolyte balance within the body, it comes as no surprise that renal abnormalities are inextricably linked with congestive heart failure. What is surprising is that recent data have indicated that renal dysfunction is a more critical determinant of mortality than heart failure itself. In The Cardiorenal Syndrome: A Clinician’s Guide to Pathophysiology and Management, experts from around the world clearly outline the current issues associated with this increasingly prevalent condition, provide invaluable insight into its pathophysiology, and highlight the risks, benefits, and rationales for both practical and promising prevention and treatment strategies, including: Diuretics, Role of ACE inhibitors/ angiotensin receptor blockers in renal dysfunction, New “designer” peptides for heart failure, Inotropes and neurohormonal blockade, Dialysis and ultrafiltration, Renal cell therapy, Left ventricular assist device implantation, and Renal and/or heart transplantation. While recognition of the role of renal dysfunction in the progression of heart failure is a pivotal advance, many questions about the scope and impact of this condition remain. This book serves as a touchstone for cardiovascular specialists and internists as they encounter the diverse varieties of cardiorenal syndrome and attempt to navigate the radically different and evolving therapeutic options in their quest to provide optimal individualized care for their patients.

Antimicrobial agents either kill (bactericidal) or inhibit (bacteriostatic) the growth of a microorganism by targeting specific unique bacterial sites or metabolic pathways. Common antibiotic adverse effects and toxicities include allergic reactions, numerous possible toxicities, alteration of human flora, drug interactions, and promoting of antibiotic resistance. Presumptive and empirical therapy relies on the clinical syndrome and anatomical site of infection. Factors to consider when choosing empirical therapy include identification of the organism, the estimated benefit to the patient, the probable susceptibility of the isolated (or suspected) pathogen based on laboratory results or local epidemiological parameters, and pharmacokinetic/pharmacodynamic considerations. Host factors, including age, immune deficiency status, and other underlying conditions, should always be considered when choosing an antibiotic. There are few absolute indications for the prophylactic use of antimicrobials, and this is one area where misuse is common. The susceptibility of common pathogens is often predictable, allowing adequate empirical treatment. The main driver for the development of antibiotic resistance is the inappropriate use of antibiotics. Bacteria may be naturally resistant or may acquire resistance by means of DNA mutation or acquisition of resistance-conferring DNA from another source (e.g. plasmids). Mechanisms of antibiotic resistance include chemical modification of antibiotics, reduced uptake into cells, active efflux from cells, target site modification, overproduction of antibiotic target, and metabolic bypass of inhibited reaction. Control of resistance may be achieved through adherence to prescribing guidelines, restriction policies that reduce the use of certain antibiotics, and antimicrobial stewardship programmes

Pages: 286 Size: 1.6 MB Format: PDF Publisher: Wiley-Blackwell
Published: 08 December, 2011

A new era of opportunity is emerging for advanced practice registered nurses (APRNs) to increasingly address gaps in access to quality healthcare. APRNs must be prepared to practice to the full scope of their education and abilities. As more and more states pass legislation enhancing prescriptive authority and fully autonomous practice for APRNs, a comprehensive resource on prescribing practices is needed. The increasing demands of today`s fast-paced healthcare environment require a new level of prescriber expertise. Knowledge about medication characteristics and effectiveness must be accompanied by an understanding of the context and process of prescribing. The Advanced Practice Registered Nurse as a Prescriber provides practicing APRNs and students with the information necessary to make fully informed, rational and ethical decisions as prescribers. This book opens with an overview of the role of the APRN prescriber and moves on to discussing practical issues such as managing difficult patient situations, special considerations when prescribing controlled substances, the influence of pharmaceutical marketing, state regulation, and legal aspects of prescribing. The book also examines barriers to prescribing, and the concluding chapter underscores key information to build cultural competence when prescribing. An evidence-based resource for all APRNs and APRN students, The Advanced Practice Registered Nurse as a Prescriber provides a comprehensive and practical resource essential for APRNs in all advanced practice roles.
