Public health nursing–with its focus on compassionate, holistic care and service to the poor, the aged, those suffering from social injustice, and those without adequate health facilities–had its origins over a century ago with the founding of the Henry Street Settlement in New York City. Embracing the same foundational principles, Nurse-Led Health Clinics is the first book to describe innovative, nurse-managed solutions for improving health care today. It addresses the key business, policy, medical, financial, and operational considerations necessary for successfully opening and operating nurse-led health facilities. With the mission to dramatically expand access to primary and preventative health care, these clinics provide a full range of services, including primary care, health promotion, disease prevention, and behavioural health care to residents of underserved communities throughout the U.S. The book delivers a wealth of comprehensive information for nurses who are considering opening their own clinics. Reinforced with best-practice models and case studies, it takes a concrete look at what it takes to successfully start and run a nurse-managed health center. The book addresses the history and growth of nurse-led clinics and describes the nurse-led paradigm of care. It identifies the different types of nurse-led clinics (primary care, school based, wellness clinics, and more) and the clinical services offered within them. Also discussed are the requirements and mind-set of potential consumers and strategies for sustainability along with the role of the collaborative team. The pros and cons of a variety of business and operations models are examined along with quality metrics and initiatives. The book also covers various state and federal policy challenges and opportunities and explores the future of nurse-led care in view of ongoing health care reform. Helpful appendices include a start-up checklist, sample by-laws and a managed care contracting toolkit. Key Features:

  • Describes key business, policy, medical, financial, and operational considerations for running a nurse-managed health center
  • Addresses the pros and cons of a variety of business models for nurse-led care
  • Identifies the most common clinical services offered

The Complete Recovery Room Book

Author: Anthea Hatfield. Pages: 593 Size: 3.48 MB Format: PDF Publisher: Oxford University Press
Published: 27 February, 2014
eISBN-13: 9780191644139

The care that a patient receives in the first hours after surgery is crucial to minimizing the risk of complications such as heart attacks, pneumonia, and blood clots. As the patient awakes from their drug-induced coma, it takes time for them to metabolize and excrete these drugs, during which period they remain unable to care for themselves, and at increased risk of harm. The recovery room staff must manage both comatose and physiologically unstable patients, and deal with the immediate post-operative care of surgical patients. The fifth edition of this popular book provides nurses, surgeons and anaesthetists with clear guidance on how to manage day-to-day problems and how to make difficult decisions. Previous editions of this book have established it as the definitive guide to setting-up, equipping, staffing, and administering an acute care unit. It includes basic science such as physiology and pharmacology, specific symptoms including pain and vomiting, and has chapters devoted to the unique post-operative needs of individual types of surgery. This new edition brings this important text up to date and new drugs and techniques for monitoring are described. A new section looks ahead to the future development and design of recovery rooms and how they can contribute to patient well being.

Care at the End of Life

Author: Edited By Jeff Round. Pages: 210 Size: 1.54 MB Format: PDF Publisher: Springer Verlag
Published: 01 January, 2016
eISBN-13: 9783319282671

This work carefully guides the reader through the methodological, policy and ethical challenges facing health economists conducting research in palliative care. It has collected the opinions of many cutting-edge researchers. Those who design and conduct economic evaluations or economics-related research in end of life populations will find this book thought provoking, instructive and informative. The provision of care to individuals with disorders associated with advancing age, such as cancer and dementia, is an increasing concern amongst policy makers and providers of health and social care. Accordingly, the burden on state and private funders in providing care to patients with these complex illnesses is of growing importance to health economists. However, answering the questions raised by the research community on end of life and palliative care health economics has received little attention. The authors shed light on many questions including: Are economic evaluation methods fit for purpose in patients at the end of life? What is the best way to measure and value health outcomes in this population? What are the appropriate societal rules to govern resource allocation for people at the end of life? Are these people more or less deserving of resources than other patients? Does age matter? How can we define a good death for the purposes of resource allocation decision making? What ethics govern research in end of life patients?

Alzheimer’s Disease and Dementia: What everyone needs to know

Size: 1.33 MB Format: PDF Publisher: Oxford University Press
Published: 02 January, 2018
eISBN-13: 978019060312

Alzheimer’s is swiftly on the rise: it is estimated that every 67 seconds, someone develops the disease. For many, the words “Alzheimer’s diseas” or “dementia” immediately denote severe mental loss and, perhaps, madness. Indeed, the vast majority of media coverage of Alzheimer’s disease (AD) and other types of dementia focuses primarily on the losses experienced by people diagnosed and the terrible burden felt by care partners yearning for a “magic bullet” drug cure. Providing an accessible, question-and-answer-format primer on what touches so many lives, and yet so few of us understand, Alzheimer’s Disease and Dementia: What Everyone Needs to Know(R) contributes what is urgently missing from public knowledge: unsparing investigation of their causes and manifestations, and focus on the strengths possessed by people diagnosed. Steven R. Sabat mines a large body of research to convey the genetic and biological aspects of Alzheimer’s disease, its clinical history, and, most significantly, to reveal the subjective experience of those with Alzheimer’s or dementia. By clarifying the terms surrounding dementia and Alzheimer’s, which are two distinct conditions, Sabat corrects dangerous misconceptions that plague our understanding of memory dysfunction and many other significant abilities that people with AD and dementia possess even in the moderate to severe stages. People diagnosed with AD retain awareness, thinking ability, and sense of self; crucially, Sabat demonstrates that there are ways to facilitate communication even when the person with AD has great difficulty finding the words he or she wants to use. From years spent exploring and observing the points of view and experiences of people diagnosed, Sabat strives to inform as well as to remind readers of the respect and empathy owed to those diagnosed and living with dementia. Alzheimer’s Disease and Dementia conveys this type of information and more, which, when applied by family and professional caregivers, will help improve the quality of life of those diagnosed as well as of those who provide support and care.

Followership: What It Takes to Lead

Author: James Shindler, James Schindler Pages: 83 Size: 1.14 MB Format: EPUB Publisher: Business Expert Press
Published: 18 November, 2014

Every leader is also a follower. Both good leaders and good followers exhibit many of the same characteristics. Both think for themselves, both are active in the leadership process, and both exhibit positive energy. Traditionally, leadership classes and leadership development programs devote little time and attention to developing effective follower skills because most organizational leaders erroneously assume that employees know how to follow. This book takes a look at both current leadership and followership theories and describes how to apply them in an organizational setting. The book also provides an overview of what it means to be a good follower and provides a roadmap of how to develop followership skills that can easily be translated to leadership skills as the need arises. Generally, there is a negative connotation of the term followership due to a lack of understanding of the topic of followership. This lack of understanding has frequently caused leaders to overlook followership as a necessary part of a successful organization. Increasing awareness of followership processes within the organizational context will lead to improved leadership and improved organizational performance. This book will aid in the development of those great followers who are great employees who in-turn will possess the skills necessary to become great leaders. This book is intended for a broad audience including both students and practitioners of Leadership.

Capitalism without Capital: The Rise of the Intangible Economy

Published: 29 November, 2017

Presents three possible scenarios for what the future of an intangible world might be like, and by outlining how managers, investors, and policymakers can exploit the characteristics of an intangible age to grow their businesses, portfolios, and economies.  This book from Nesta’s 8 of the best books on innovation reading list reports how early in the twenty-first century, a quiet revolution occurred. For the first time, the major developed economies began to invest more in intangible assets, like design, branding, R&D, and software, than in tangible assets, like machinery, buildings, and computers. For all sorts of businesses, from tech firms and pharma companies to coffee shops and gyms, the ability to deploy assets that one can neither see nor touch is increasingly the main source of long-term success. It shows that the growing importance of intangible assets has also played a role in some of the big economic changes of the last decade. The rise of intangible investment is, Jonathan Haskel and Stian Westlake argue, an underappreciated cause of phenomena from economic inequality to stagnating productivity.

This completely revised and enlarged 2nd edition of The Tobacco Epidemic provides a comprehensive update of the clinical, public health and political aspects of tobacco smoking. Since its 1st edition in 1997, knowledge on the health hazards of tobacco and nicotine addiction has increased considerably, but recent data has shown that the global problem has become more aggravated in low- and middle-income countries: if current trends continue, tobacco smoking will be responsible for the deaths of 1 billion people in the 21st century. Written by outstanding international experts, the book covers the history of tobacco production and use, the economics of tobacco use and control, as well as the health consequences of active and passive smoking in both adults and children. Special chapters discuss the impact of media, movies and TV on tobacco consumption in young people, the patterns and predictors of smoking cessation in the general population and in different social subgroups, and initiatives supported by the WHO Framework Convention on Tobacco Control. Readers will find the latest information on how nicotine dependence is treated with nicotine replacement products, what role health care professionals play in helping smokers to quit and what effects smoke-free environments, advertising bans and price increases have on smoking prevalence. The potential harms and benefits of smokeless tobacco, waterpipe tobacco smoking and electronic cigarettes are also evaluated. This book is a must-read for anyone in the medical profession who treats patients with smoking-related diseases and for those engaged in tobacco control. It will also be appreciated by interested nonmedical readers like journalists and legislators.

Published: 11 May, 2015
eISBN-13: 9780702066504

Since the third edition of Grieve’s Modern Manual Therapy was published in 2005, the original concepts of manipulative therapy have grown to embrace new research-generated knowledge. Expansions in practice have adopted new evidence which include consideration of psychological or social moderators. The original manual therapy or manipulative therapy approaches have transformed into musculoskeletal physiotherapy and this is recognized by the change in title for the new fourth edition – Grieve’s Modern Musculoskeletal Physiotherapy. Grieve’s Modern Musculoskeletal Physiotherapy continues to bring together the latest state-of-the-art research, from both clinical practice and the related basic sciences, which is most relevant to practitioners. The topics addressed and the contributing authors reflect the best and most clinically relevant contemporary work within the field of musculoskeletal physiotherapy. With this as its foundation and a new six-strong editorial team at its helm, the fourth edition now expands its focus from the vertebral column to the entire musculoskeletal system. For the first time both the spine and extremities are covered, capturing the key advances in science and practices relevant to musculoskeletal physiotherapy. The book is divided into five parts containing multiple sections and chapters. The first part of the book looks at advances in the sciences underpinning musculoskeletal physiotherapy practice. Here there is commentary on topics such as movement, the interaction between pain and motor control as well as neuromuscular adaptations to exercise. Applied anatomical structure is covered in addition to the challenges of lifestyle and ageing. A new section highlights the important area of measurement and presents the scope of current and emerging measurements for investigating central and peripheral aspects relating to pain, function and morphological change. Another section discusses some contemporary research approaches such as quantitative and qualitative methods as well as translational research. Part III contains sections on the principles of and broader aspects of management which are applicable to musculoskeletal disorders of both the spine and periphery. Topics include models for management prescription, communication and pain management and contemporary principles of management for the articular, nervous and sensorimotor systems. In recognition of the patient centred and inclusive nature of contemporary musculoskeletal practice, there is also discussion about how physiotherapists may use cognitive behavioural therapies when treating people with chronic musculoskeletal disorders. The final part of the book focuses on selected contemporary issues in clinical practice for a particular region, condition or the most topical approaches to the diagnosis and management of a region. A critical review of the evidence (or developing evidence) for approaches is given and areas for future work are highlighted.

or the first time, a top leadership scholar and a top leadership practitioner explore the true duties, demands, and privileges of leadership. Intellectual sparks flew when Warren Bennis, the father of modern leadership studies and Steven B. Sample, one of the most accomplished university presidents in recent history, came together for candid explorations of the forces that shape successful leaders and unsuccessful ones. The Art and Adventure of Leadership, their final collaboration, reveals the profound insights that the authors gained together over the 16 years in which they co-taught one of the most popular leadership courses in America. Here, each brings his own distinct vantage point as they address the mechanics and mysteries of leadership. The result is a unique examination of the journey of great leaders from momentary setbacks to ultimate success. It offers profound lessons on what determines the difference between failure and redemption for leaders. And it illuminates important and overlooked dimensions of great leaders ranging from Winston Churchill to Steve Jobs. Together, they explore why:

  • A mature leader must grasp when it’s healthy to risk failure, and when failure can’t be tolerated at any cost
  • Leadership isn’t for everyone and requires a particular set of skills and competencies that are often glossed over in most management literature
  • To succeed in an uncertain and fast-changing world, a shrewd leader must understand which aspects of human society change and which aspects never change
  • A mature, wise leader must seek a balance between high-minded ideals and the gritty realities and compromises that leaders face in their daily lives
  • Above all, meaningful leadership remains a matter of character

With incredible insight, this book examines why George Washington, Abraham Lincoln, and other giants were able to recover from failures, learn resilience, and prepare themselves for their moments of destiny. In so doing, it demonstrates and helps cultivate the leadership skills that you need to create your own most meaningful legacy. The Art and Adventure of Leadership is a unique look at leadership, and a critical resource for the leaders of tomorrow.

Diagnostic Clusters in Shoulder Conditions

Author: Puneet Monga, Lennard Funk
Pages: 277 Size: 5.51 MB Format: PDF Publisher: Springer Verlag
Published: 01 January, 2017
eISBN-13: 9783319573342

This book serves as a definitive guide to diagnosing shoulder conditions for all levels of orthopaedic surgeon with an interest in shoulder pathology, and for junior surgeons in training. It comes at a time when the knowledge regarding shoulder conditions has vastly improved, with the shoulder sub-speciality growing at a rapid pace in terms of practitioner numbers, procedures and evidence-base. However, with the multitude of special tests for shoulder conditions, the clinician faces a variety of sensitivities and specificities of the respective tests. It is unclear, in most circumstances, which single test is the best and growing evidence confirms that a set of tests is superior for diagnosing shoulder conditions. This forms the basis of cluster testing, which is the key concept for the title and content of this book. In many common shoulder diagnoses the preference for clusters has been reflected in both experience and clinical evidence, but for conditions where evidence for clusters is less clear, a consensus-based approach is utilised by the authors of this key resource for diagnosing shoulder conditions.

This new edition of the Oxford Handbook has been developed for anyone interested in the hand—surgeon, therapist, nurse, rheumatologist, general practitioner, and emergency practitioner. This reflects the broad nature of the speciality in a multidisciplinary world where surgery is by no means the only approach. The authors have all prepared the book free of charge—except for a complimentary copy of the book. The editors will donate all royalties to charity. We hope to give something back to the speciality that has brought us so much professional satisfaction. The format of the book—carefully organized headings and bullet lists and tables—should help the reader to take an overview of any topic as both a primer and as a revision aid, as well as to delve deeper to find the details of a complex classification. The small size of the book—readily accessible in a pocket or briefcase or bag or desk drawer—makes this rarely used and always forgotten knowledge easy to access.

In some ways, movement disorders are amongst the most obvious of neurological disorders; without any special examination techniques beyond simple observation, most movement disorders can be readily appreciated. But how can one take these basic observations and translate them into diagnostic formulations, appropriate investigation, and even treatment? In a specialty where there are few diagnostic tests, and where lists of differential diagnoses of the six main movement disorders seem to expand every week, how does the clinician approach the patient with a movement disorder? This book is designed to guide the reader through this process. This book attempts to build a route from careful targeted history taking and examination through to appropriate investigation and practical treatment advice. Although focus has been kept on the commoner movement disorders, in particular Parkinson’s disease, it has also been ensured that the book is comprehensive by including details of rarer movement disorders, highlighting their clinical features and explaining when they should be considered in the differential diagnosis.

Common antibiotic adverse effects and toxicities include allergic reactions, numerous possible toxicities, alteration of human flora, drug interactions, and promoting of antibiotic resistance. Presumptive and empirical therapy relies on the clinical syndrome and anatomical site of infection. Factors to consider when choosing empirical therapy include identification of the organism, the estimated benefit to the patient, the probable susceptibility of the isolated (or suspected) pathogen based on laboratory results or local epidemiological parameters, and pharmacokinetic/pharmacodynamic considerations. Host factors, including age, immune deficiency status, and other underlying conditions, should always be considered when choosing an antibiotic. There are few absolute indications for the prophylactic use of antimicrobials, and this is one area where misuse is common. The susceptibility of common pathogens is often predictable, allowing adequate empirical treatment. The main driver for the development of antibiotic resistance is the inappropriate use of antibiotics. Bacteria may be naturally resistant or may acquire resistance by means of DNA mutation or acquisition of resistance-conferring DNA from another source (e.g. plasmids). Mechanisms of antibiotic resistance include chemical modification of antibiotics, reduced uptake into cells, active efflux from cells, target site modification, overproduction of antibiotic target, and metabolic bypass of inhibited reaction. Control of resistance may be achieved through adherence to prescribing guidelines, restriction policies that reduce the use of certain antibiotics, and antimicrobial stewardship programmes
